By completing the registration & Indemnity Release form of “TANDEM PARAGLIDING PASSENGER” , the client whose names mentioned above are contractually obliged to comply and agrees to the following terms with the declaration of “I hereby declares that…” :-

1. I am good health condition to engage myself in this activity and will comply with the safety rules and not persuaded or forced in any way, as it is entirely my decision to participate
2. I agree to release any indemnity claim from the paragliding operator, the tandem pilot and any registration organizer for any inflicted injury or fatality caused from an accident during the activity;
3. I declare that I am fully aware of the inherent risks from this activity and have taken all possible measures to ensure that I am compliant to the safety myself including others that participates in this activity;
4. I agree to grant the service provider the permission to use visual contents created during the activity for the purpose of record keeping, promotional materials and as evidence during investigations of any uneventful incident or accident;
5. I agree to subscribe to the INTERNAL PROTECTION COVERAGE provided by Cloud Bass Enterprise (KKB Paragliding Park Operator) and all medical expenses related to the accident or incident in Tandem Paragliding activity are under the responsibility of the operator at the rate prescribed;

6. I agree to be subjected to an additional charges for reaching or exceeding the weight limit of 88kg, at the amount of RM50 for each weight-limit violation.
